Overview

**ipragliflozin + insulin** is a combination therapy utilizing the sodium–glucose cotransporter 2 (SGLT2) inhibitor ipragliflozin with exogenous insulin for the treatment of diabetes mellitus, specifically in patients with inadequate glycemic control on insulin alone. Ipragliflozin promotes urinary glucose excretion by inhibiting SGLT2 in the renal proximal tubules, thereby reducing blood glucose levels independently of insulin action. Insulin, a peptide hormone, facilitates glucose uptake in peripheral tissues and suppresses hepatic gluconeogenesis. The combination is effective in both type 1 and type 2 diabetes with evidence showing significant and sustained reduction in HbA1c, daily insulin dose, and body weight, with an acceptable safety profile. The primary indication is diabetes mellitus where insulin alone does not adequately achieve glycemic targets. The combination is not a fixed-dose product but a therapeutic regimen used in clinical practice and studied in formal clinical trials[2][3].
