Overview

Ipratropium bromide + theophylline is a combination therapy used primarily in the management of chronic obstructive pulmonary disease (COPD) and sometimes asthma. Ipratropium bromide is an inhaled anticholinergic bronchodilator that works by blocking muscarinic receptors in airway smooth muscle, leading to bronchodilation. Theophylline is an oral methylxanthine bronchodilator that acts as a non-selective phosphodiesterase inhibitor, increasing intracellular cAMP and promoting relaxation of bronchial smooth muscle. The combination leverages their distinct mechanisms for additive or synergistic improvement in lung function and exercise capacity, with studies showing greater increases in FEV1 and exercise tolerance compared to either drug alone[1][2][3][4]. This combination may be considered when patients do not adequately respond to monotherapy or cannot tolerate beta agonists.
