Irbesartan + trandolapril is a combination therapy investigated by the Ottawa Hospital Research Institute for the treatment of diabetic nephropathy. It consists of irbesartan, an angiotensin receptor blocker (ARB), and trandolapril, an angiotensin-converting enzyme (ACE) inhibitor. This combination aims to provide a more complete blockade of the renin-angiotensin-aldosterone system (RAAS) than monotherapy. Trandolapril works by inhibiting the ACE enzyme, which prevents the conversion of angiotensin I to the potent vasoconstrictor angiotensin II. Irbesartan works by selectively blocking the binding of angiotensin II to the AT1 receptor. In the context of diabetic nephropathy, this dual mechanism is intended to reduce intraglomerular pressure and proteinuria, thereby slowing the progression of renal damage.
