Irinotecan hydrochloride liposome (marketed as Onivyde) is a topoisomerase 1 inhibitor encapsulated in a lipid bilayer vesicle (liposome), designed to enhance delivery and prolong exposure of the active drug and its metabolite SN-38 to tumor cells. It is used in combination with fluorouracil (a pyrimidine analog antimetabolite that inhibits thymidylate synthase) and leucovorin (which enhances the effects of fluorouracil) for the treatment of metastatic pancreatic adenocarcinoma, particularly after progression on gemcitabine-based therapy or as first-line therapy with additional agents. The mechanism involves inhibition of DNA replication by stabilizing topoisomerase I-DNA complexes, leading to double-strand DNA breaks and cell death.
