NALIRI-VT is an investigational combination chemotherapy regimen comprising irinotecan hydrochloride liposome injection, temozolomide, and vincristine. Developed under the leadership of Tang Xiaodong for the treatment of advanced Ewing's sarcoma, this regimen utilizes a nanoliposomal formulation of irinotecan to improve the drug's pharmacokinetic profile and intratumoral concentration. Irinotecan acts by inhibiting DNA topoisomerase I, leading to lethal DNA strand breaks during replication. Temozolomide, an oral alkylating agent, further contributes to DNA damage by methylating guanine residues. Vincristine, a vinca alkaloid, disrupts microtubule assembly, causing mitotic arrest. This triplet combination is currently being evaluated in a randomized Phase 2 clinical trial (NALIRI-VT vs. VIT) to determine if the liposomal formulation of irinotecan offers superior efficacy and safety compared to standard irinotecan in patients who have failed first-line therapy.
