Irinotecan liposomes + capecitabine is a combination chemotherapy regimen being investigated for the neoadjuvant treatment of locally advanced rectal cancer (LARC). Irinotecan liposome is a nanoparticle formulation of irinotecan, a topoisomerase I inhibitor, encapsulated in a lipid bilayer vesicle to enhance tumor delivery and prolong systemic circulation. Capecitabine is an oral fluoropyrimidine prodrug that is enzymatically converted to 5-fluorouracil (5-FU) in the body, where it inhibits thymidylate synthase. In a Phase I study led by Zhejiang Cancer Hospital, this combination is administered concurrently with radiotherapy. A key feature of this regimen's development is the use of genotype-driven dosing, where the dose of irinotecan liposomes is stratified based on the patient's UGT1A1 *28 and *6 polymorphisms to optimize safety and manage potential dose-limiting toxicities such as neutropenia and diarrhea.
