This is a combination chemotherapy regimen consisting of three agents: - **Irinotecan liposomes** is a nanoliposomal formulation of irinotecan, a topoisomerase I inhibitor. Encapsulation in liposomes prolongs circulation time, enhances tumor accumulation via the enhanced permeability and retention (EPR) effect, and improves pharmacokinetics compared to free irinotecan. Irinotecan is converted to its active metabolite SN-38, which inhibits topoisomerase I, leading to DNA damage and cell death. - **Cisplatin** is a platinum-based chemotherapeutic that forms DNA crosslinks and induces apoptosis by interfering with DNA replication and transcription. - **Carboplatin** is another platinum-based agent similar in mechanism to cisplatin but with different pharmacokinetic properties. This combination leverages the cytotoxic effects of all three drugs for synergistic antitumor activity. The regimen has been explored as an option for first-line treatment in extensive-stage small-cell lung cancer (ES-SCLC) and neuroendocrine carcinoma[1][2][3]. Liposomal formulations are designed to reduce toxicity while maintaining or enhancing efficacy.
