Overview

The combination of isatuximab, carfilzomib, lenalidomide, and dexamethasone (often abbreviated as Isa-KRd) is a four-drug regimen used primarily for the treatment of high-risk newly diagnosed multiple myeloma. Isatuximab is a monoclonal antibody targeting CD38 on plasma cells; carfilzomib is a second-generation proteasome inhibitor that disrupts protein degradation in cancer cells; lenalidomide is an immunomodulatory agent with anti-myeloma and antiangiogenic effects; and dexamethasone is a corticosteroid that reduces inflammation and suppresses immune responses. This quadruple regimen has shown promising efficacy in clinical trials for both transplant-eligible and non–transplant eligible patients with high-risk genetic features. The primary mechanism involves direct cytotoxicity to myeloma cells, immune modulation, inhibition of proteasome activity leading to apoptosis, and reduction of tumor-associated inflammation[1][4][5].
