isatuximab + dexamethasone

isatuximab + dexamethasone (Isa-dex) is a combination therapy primarily utilized for the treatment of relapsed or refractory multiple myeloma. Isatuximab is a chimeric IgG1 monoclonal antibody that targets a specific epitope on the CD38 receptor, which is highly expressed on the surface of multiple myeloma cells. It induces cell death through multiple pathways, including antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), complement-dependent cytotoxicity (CDC), and the direct induction of apoptosis. Dexamethasone is a potent synthetic glucocorticoid that acts as an agonist of the glucocorticoid receptor, providing direct lympholytic effects and enhancing the antitumor activity of isatuximab while helping to manage infusion-related reactions. This doublet regimen is frequently used as a backbone for various triplet combinations in clinical practice.
