Overview

Isatuximab + iberdomide + bortezomib + dexamethasone is an investigational quadruplet combination regimen being developed primarily by the Dana-Farber Cancer Institute for the treatment of newly diagnosed multiple myeloma (NDMM). This regimen is being evaluated in both transplant-eligible and transplant-ineligible patients, notably within the DETERMINATION-2 trial. The combination leverages four distinct mechanisms: isatuximab is an anti-CD38 monoclonal antibody that induces myeloma cell death; iberdomide is a next-generation cereblon E3 ligase modulator (CELMoD) that promotes the degradation of Ikaros and Aiolos transcription factors; bortezomib is a proteasome inhibitor that disrupts protein homeostasis; and dexamethasone is a corticosteroid that provides synergistic anti-inflammatory and anti-myeloma effects. This quadruplet represents a next-generation induction strategy aimed at improving deep response rates in frontline myeloma therapy.
