Combination therapy of **isatuximab** (a monoclonal antibody targeting CD38), **lenalidomide** (an immunomodulatory agent), and **dexamethasone** (a synthetic glucocorticoid corticosteroid) is used primarily in the treatment of relapsed/refractory multiple myeloma. Isatuximab is a chimeric IgG1 monoclonal antibody that binds to a unique epitope on CD38, inducing several antitumor mechanisms, including direct induction of apoptosis, antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), complement-dependent cytotoxicity, and inhibition of CD38 ecto-enzymatic activity[1][2][4][6]. Lenalidomide enhances immune effector cell activity, especially natural killer and T cells, and exerts direct anti-proliferative effects on myeloma cells. Dexamethasone has anti-inflammatory and lympholytic properties, suppressing tumor cell proliferation and enhancing the effects of isatuximab and lenalidomide. This three-drug regimen, via synergistic immunostimulatory and cytotoxic mechanisms, results in increased efficacy in heavily pretreated multiple myeloma patients, with an overall favorable safety profile and significant objective response rates[1][5].
