Overview
Isatuximab + pomalidomide + dexamethasone (IPd) is a combination therapy regimen being evaluated for the treatment of AL (light chain) amyloidosis. The regimen consists of isatuximab, a monoclonal antibody that targets the CD38 receptor on plasma cells; pomalidomide, an immunomodulatory small molecule that binds to cereblon; and dexamethasone, a corticosteroid. This triplet therapy is primarily being studied in the Phase 2 ISAMYP trial, sponsored by the Intergroupe Francophone du Myelome (IFM) in collaboration with the Australasian Leukaemia & Lymphoma Group (ALLG). The regimen is intended for patients who have not achieved a very good partial response (VGPR) or better after previous therapy or those who have relapsed. Isatuximab induces cell death through multiple mechanisms including antibody-dependent cellular cytotoxicity (ADCC) and direct apoptosis, while pomalidomide provides immunomodulatory and antiproliferative effects.
