Overview

ISIS-GCGRRx + metformin is a combination treatment for type 2 diabetes mellitus in patients whose blood sugar is uncontrolled with metformin monotherapy. ISIS-GCGRRx (also referred to as ISIS 449884 or IONIS GCGRRx) is a 2′-O-methoxyethyl antisense oligonucleotide that targets the glucagon receptor (GCGR) to reduce hepatic glucose output and improve glycemic control[3][5][1]. By inhibiting GCGR, it blocks the action of glucagon, decreasing glucose production by the liver. This mechanism leads to increased circulating GLP-1, a hormone that supports pancreatic function and enhances insulin secretion[1][3]. When used in combination with metformin, ISIS-GCGRRx has demonstrated clinically meaningful reductions in HbA1c and improved glycemic control with an acceptable safety profile in phase II clinical trials for patients with type 2 diabetes[3][5]. Metformin is a well-established oral antihyperglycemic drug that improves insulin sensitivity and lowers hepatic glucose production.
