Islet + mesenchymal stem cell refers to the co-transplantation of pancreatic islets with mesenchymal stem cells (MSCs) as a therapeutic approach primarily for type 1 diabetes mellitus (T1DM). This combination aims to address the limitations of traditional islet transplantation, such as poor graft survival and immune rejection. Pancreatic islets provide insulin-producing β cells, while MSCs—derived from sources like bone marrow, adipose tissue, or umbilical cord blood—offer immunomodulatory effects, promote angiogenesis and tissue repair, and secrete regenerative factors that support graft viability. The mechanism involves MSCs protecting transplanted β cells from immune-mediated damage and enhancing their function by creating a supportive microenvironment. Clinical studies have shown that this strategy can improve glycemic control without serious adverse effects; however, efficacy in T1DM remains under investigation with ongoing clinical trials[4][5][2].
