Overview

Isocarboxazid + trimipramine is a combination of two antidepressant drugs, each with distinct mechanisms of action. Isocarboxazid is a nonselective, irreversible monoamine oxidase inhibitor (MAOI) that increases the levels of serotonin, norepinephrine, dopamine, and epinephrine in the central nervous system by inhibiting their breakdown via monoamine oxidase enzymes. Trimipramine is a tricyclic antidepressant (TCA) that primarily acts as an antagonist at various neurotransmitter receptors and inhibits the reuptake of norepinephrine and serotonin to a lesser extent than other TCAs. This combination has been studied for treatment-resistant depression; some clinical reports suggest enhanced efficacy and faster onset compared to monotherapy with either agent alone. However, this combination carries significant risk due to potential pharmacodynamic interactions—most notably an increased risk for serotonin syndrome or hypertensive crisis—and is generally contraindicated or not recommended in modern practice due to safety concerns[1][5][7].
