Overview

Isolyte + lactated ringers refers to a combination or sequential administration strategy of two balanced crystalloid solutions used for intravenous fluid resuscitation. **Isolyte** (specifically Isolyte S) is a buffered isotonic multi-electrolyte injection containing sodium, potassium, magnesium, chloride, acetate, and gluconate. **Lactated Ringer's** (LR) is a standard balanced solution containing sodium, potassium, calcium, chloride, and lactate. These fluids are termed "balanced" because their electrolyte composition and strong ion difference (SID) more closely approximate human plasma compared to 0.9% normal saline. The inclusion of bicarbonate precursors (lactate in LR; acetate and gluconate in Isolyte) helps maintain acid-base equilibrium and prevents the development of hyperchloremic metabolic acidosis, a common complication of large-volume resuscitation with high-chloride fluids. Clinically, these fluids are primarily utilized in the management of sepsis, trauma, and major surgical procedures to restore extracellular fluid volume and maintain hemodynamic stability.
