9HLE is a 9-month anti-tubercular treatment regimen comprising isoniazid, levofloxacin, and ethambutol. Developed by the Institute of Liver and Biliary Sciences in India, it is specifically intended for treating tuberculosis in patients with underlying compensated or decompensated chronic liver disease. By replacing potentially hepatotoxic agents like rifampicin and pyrazinamide with levofloxacin, the regimen aims to provide an effective therapeutic option while minimizing the risk of drug-induced liver injury. Isoniazid acts by inhibiting mycolic acid synthesis, levofloxacin targets bacterial DNA gyrase and topoisomerase IV, and ethambutol interferes with cell wall assembly by inhibiting arabinosyl transferase.
