Overview

2HPM is a three-drug combination regimen used for the treatment of tuberculosis (TB), specifically designed to shorten the duration of therapy from the standard six months to four months. The regimen consists of isoniazid, a nicotinic acid derivative that inhibits mycolic acid synthesis; moxifloxacin, a fourth-generation fluoroquinolone that targets bacterial DNA gyrase and topoisomerase IV; and rifapentine, a long-acting rifamycin that inhibits DNA-dependent RNA polymerase. This combination was a key component of the landmark Study 31/A5241 trial, which demonstrated the non-inferiority of a four-month rifapentine-based regimen compared to the standard six-month treatment. In the context of the NIAID-sponsored ACTG A5406 trial, 2HPM is being investigated for its safety and pharmacokinetic profile when used alongside high-dose dolutegravir in patients with HIV-associated tuberculosis.
