Overview

Empiric TB treatment, in the context of the PrOMPT (Presumptive TB Treatment in HIV-infected Patients Initiating ART) trial, refers to the presumptive administration of the standard four-drug anti-tuberculosis regimen—comprising isoniazid, rifampicin, pyrazinamide, and ethambutol—to severely immunocompromised HIV-infected patients starting antiretroviral therapy (ART). This approach was investigated to reduce the high early mortality rates associated with undiagnosed tuberculosis in sub-Saharan Africa. The PrOMPT trial, led by Prof. JMA Lange, was a Phase 4 study that compared this systematic empiric treatment against test-guided therapy in patients with very low CD4 counts (<50 cells/μL). The study concluded that systematic empiric TB treatment did not significantly reduce mortality compared to the standard of care and was associated with a higher incidence of adverse events.
