Overview

2HPMZ + 2HPM (also known as 2HPMZ/2HPM) is a four-month, short-course antibiotic combination regimen investigated for the treatment of drug-susceptible pulmonary tuberculosis. The regimen consists of an intensive phase of two months of daily isoniazid (H), rifapentine (P), moxifloxacin (M), and pyrazinamide (Z), followed by a continuation phase of two months of daily isoniazid, rifapentine, and moxifloxacin. This regimen is designed as a shorter alternative to the standard six-month 2HRZE/4HR treatment. The components work synergistically: isoniazid inhibits mycolic acid synthesis; rifapentine is a rifamycin-class RNA polymerase inhibitor; moxifloxacin is a fluoroquinolone that inhibits DNA gyrase and topoisomerase IV; and pyrazinamide is active against semi-dormant bacilli. It is currently being evaluated in clinical trials, including studies sponsored by the Shanghai Public Health Clinical Center specifically for HIV-infected patients with low CD4 counts.
