Overview

**Isosorbide + betahistine** is a combination of two small molecule drugs: isosorbide, an osmotic diuretic, and betahistine, a histamine-like antivertigo agent. Isosorbide acts as an osmotic diuretic to reduce inner ear fluid overload, leveraging its ability to decrease endolymphatic pressure in the inner ear, and is sometimes used in the management of Ménière's disease[3][4][5]. Betahistine functions primarily as a histamine H1-receptor agonist and H3-receptor antagonist; it increases cochlear blood flow and reduces endolymphatic pressure as well, providing symptomatic relief of vertigo in Ménière's disease[1][4][5]. This combination seeks to target both the fluid overload and the vestibular symptoms in the disease, though evidence for the benefit of combination therapy is limited and based on small clinical studies[5].
