Overview

**Isosorbide dinitrate + hydralazine** is an oral fixed-dose vasodilator combination comprising the nitric-oxide-donor nitrate isosorbide dinitrate and the direct arteriolar vasodilator hydralazine hydrochloride. Marketed in the United States as **BiDil**, the 20 mg isosorbide dinitrate/37.5 mg hydralazine hydrochloride tablet is approved as an adjunct to standard therapy for heart failure in self-identified Black patients, to improve survival, prolong time to heart-failure hospitalization, and improve patient-reported functional status. Isosorbide dinitrate produces venous and arterial dilation through nitric oxide signaling, reducing preload and afterload; hydralazine predominantly dilates arterioles, reducing systemic vascular resistance and afterload, and may limit nitrate tolerance.
