Overview

ITM + bupivacaine QLB is a combination analgesic regimen used primarily for postoperative pain management, especially after cesarean section and other abdominal surgeries. It combines intrathecal morphine (ITM), an opioid administered into the spinal fluid for long-lasting central analgesia, with a quadratus lumborum block (QLB) using the local anesthetic bupivacaine. The QLB is performed under ultrasound guidance to deliver bupivacaine near the quadratus lumborum muscle, providing regional anesthesia by blocking somatic and visceral nerve fibers in the abdominal wall. This multimodal approach aims to enhance pain relief while reducing systemic opioid requirements and their associated side effects[1][2][3].
