Ivonescimab is a first-in-class bispecific antibody that simultaneously targets programmed cell death protein 1 (PD-1) and vascular endothelial growth factor (VEGF). This therapeutic regimen combines ivonescimab with short-course hypofractionated radiotherapy (prescribed at 20 Gy in 5 fractions) as a second-line treatment for patients with locally advanced or metastatic esophageal squamous cell carcinoma (ESCC). Developed by Akeso Biopharma, ivonescimab is designed to enhance anti-tumor activity by blocking immune checkpoint evasion while simultaneously inhibiting tumor angiogenesis. The combination with radiotherapy, currently being evaluated in a Phase II clinical study at Anhui Provincial Hospital, aims to leverage the local cytotoxic and potentially immunogenic effects of radiation to improve outcomes in ESCC patients who have progressed on prior first-line therapies.
