Jessner's solution is a classic superficial-to-medium depth chemical peeling agent widely used in dermatology for the treatment of various skin conditions, including hyperpigmentation, acne, and photoaging. The traditional formulation, developed by dermatologist Dr. Max Jessner, consists of a synergistic combination of 14% salicylic acid, 14% lactic acid, and 14% resorcinol in an ethanol base. This combination was designed to maximize efficacy while minimizing the toxicity and side effects associated with high concentrations of its individual components. A 'Modified Jessner's solution' replaces resorcinol with citric acid (often 8% citric, 17% lactic, and 17% salicylic acid) to reduce the risk of allergic reactions and post-inflammatory hyperpigmentation, particularly in patients with darker skin types (Fitzpatrick IV-VI). The solution works by inducing keratolysis and controlled epidermal necrosis, which facilitates the removal of melanin-containing keratinocytes and promotes dermal collagen remodeling. It is frequently used as a standalone treatment or in combination with other agents like trichloroacetic acid (TCA) to enhance penetration and outcomes.
