KarXT + lecanemab is a combination therapy regimen being investigated for the treatment of Alzheimer's disease. KarXT is a co-formulation of xanomeline, a dual M1/M4-preferring muscarinic acetylcholine receptor agonist, and trospium chloride, a peripherally restricted muscarinic antagonist designed to mitigate peripheral cholinergic side effects. Lecanemab is a humanized IgG1 monoclonal antibody that selectively binds to soluble amyloid-beta (Aβ) protofibrils, promoting their clearance from the brain. This combination approach aims to provide symptomatic relief of cognitive and neuropsychiatric symptoms via muscarinic modulation while simultaneously addressing the underlying amyloid pathology of the disease. The Neurology Office of South Florida is conducting a Phase II study to evaluate the safety and tolerability of adding KarXT to patients already receiving lecanemab infusions.
