Overview

Ketamine + hydromorphone is a combination of two analgesic agents used primarily for the management of moderate to severe pain, especially in perioperative and postoperative settings. Hydromorphone is a potent opioid agonist that acts mainly on mu-opioid receptors to provide analgesia by altering the perception and response to pain in the central nervous system. Ketamine is an N-methyl-D-aspartate (NMDA) receptor antagonist with dissociative anesthetic properties; it provides analgesia by inhibiting central sensitization and reducing opioid tolerance or hyperalgesia. The combination is often used as part of intravenous patient-controlled analgesia (IV-PCA) regimens, where ketamine serves as an adjuvant to reduce total opioid requirements, mitigate opioid-induced side effects such as pruritus, and potentially improve pain control without increasing adverse events[1][4][8]. This approach may be particularly beneficial for patients with high postoperative pain or those who are tolerant or resistant to opioids.
