Overview

Ketoconazole + tamsulosin is a **combination of two small molecule drugs**: ketoconazole, an imidazole antifungal agent, and tamsulosin, a selective alpha-1 adrenergic receptor antagonist. Ketoconazole inhibits fungal cell membrane synthesis by blocking cytochrome P450 14α-demethylase and also acts as a strong inhibitor of CYP3A4, thus profoundly affecting the metabolism of drugs metabolized by this pathway, including tamsulosin. Tamsulosin works by selectively blocking alpha-1A adrenergic receptors in the prostate and bladder neck, leading to relaxation of smooth muscle and improved urine flow in conditions like benign prostatic hyperplasia (BPH). The combination has been studied for management of acute urinary retention (AUR) due to benign prostatic obstruction (BPO), where the androgen-lowering effects of ketoconazole (rapidly decreasing testosterone) may enhance the efficacy of tamsulosin in enabling successful trial without catheter (TWOC)[2][3].
