ketorolac + lidocaine is a combination analgesic and local anesthetic regimen being developed by Rush University Medical Center for the management of procedural pain, specifically during the placement of osmotic dilators for cervical preparation prior to second-trimester abortion. The regimen is administered as a paracervical block and consists of lidocaine hydrochloride, which provides rapid local anesthesia by blocking voltage-gated sodium channels and inhibiting nerve impulse conduction, and ketorolac, a nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes. By reducing prostaglandin synthesis at the site of tissue manipulation, ketorolac is intended to enhance and prolong the anesthetic effect of lidocaine. This combination is currently being evaluated in Phase 2 clinical trials (such as the PCB-KinD study) to determine its efficacy in reducing perceived pain compared to standard lidocaine-only blocks.
