Keytruda (pembrolizumab) in combination with cinrebafusp alfa (PRS-343) is an investigational immunotherapy regimen developed for HER2-positive advanced solid tumors. Pembrolizumab is a humanized monoclonal antibody that blocks the interaction between PD-1 and its ligands (PD-L1 and PD-L2), thereby releasing PD-1 pathway-mediated inhibition of the immune response, including the anti-tumor immune response. Cinrebafusp alfa is a first-in-class bispecific Anticalin-antibody fusion protein designed to target HER2 and provide localized 4-1BB (CD137) agonism. By binding to HER2-expressing tumor cells, cinrebafusp alfa clusters 4-1BB on tumor-infiltrating lymphocytes, leading to targeted T-cell activation. The combination therapy was evaluated in Phase 1 trials to determine if the synergy between checkpoint inhibition and localized costimulation could improve outcomes in patients with HER2-positive breast, gastric, and bladder cancers.
