KRAS G12V-reactive T cells (Providence Health & Services) is an autologous T-cell receptor (TCR) engineered T-cell therapy developed by the Earle A. Chiles Research Institute at Providence Health & Services. The therapy involves the genetic modification of a patient's own T cells to express a TCR specific for the KRAS G12V neoantigen, a common driver mutation in various epithelial cancers. This recognition is restricted by the HLA-DRB1*07:01 allele. The treatment protocol typically involves lymphodepleting chemotherapy with cyclophosphamide and gemcitabine, followed by the infusion of the engineered T cells and subsequent administration of the PD-1 inhibitor pembrolizumab to maintain T-cell activity. It is currently being evaluated under an intermediate-size patient population IND for patients with advanced or metastatic KRAS G12V-mutant epithelial cancers, including pancreatic adenocarcinoma.
