KRN330 + irinotecan is a combination therapy consisting of KRN330, a fully human recombinant monoclonal antibody targeting the A33 surface differentiation antigen, and irinotecan, a topoisomerase I inhibitor. The A33 antigen is expressed in approximately 95% of colorectal cancers. This combination was investigated as a second-line treatment for patients with metastatic colorectal cancer (mCRC) who had progressed after first-line therapies such as FOLFOX or CapOx. In clinical studies, the maximum tolerated dose for the combination was established at 0.5 mg/kg weekly for KRN330 with standard dosing of irinotecan (180 mg/m²). The most common toxicities were gastrointestinal effects and neutropenia. Efficacy endpoints such as objective response rate and disease control rate were modest; further development was discontinued after phase 1/2 trials did not meet prespecified efficacy criteria[2][4][6].
