This is a combination drug consisting of five active ingredients: - **L-carnitine** is a naturally occurring amino acid derivative involved in energy metabolism and fatty acid transport. - **Eicosapentaenoic acid (EPA)** is an omega-3 fatty acid with anti-inflammatory properties, commonly found in fish oil. - **Medroxyprogesterone** is a synthetic progestin used primarily for hormonal therapy, contraception, and certain cancers. - **Megestrol acetate** is a synthetic progestin used as an appetite stimulant (notably for cachexia/wasting syndromes) and as an antineoplastic agent in breast and endometrial cancer. It acts mainly as an agonist of the progesterone receptor[1][2][3][4][5]. - **Thalidomide** is an immunomodulatory agent with anti-inflammatory and antiangiogenic effects. It inhibits tumor necrosis factor-alpha (TNF-alpha), modulates cytokines such as IL-6, inhibits angiogenesis via basic fibroblast growth factor (bFGF) and vascular endothelial growth factor (VEGF), and binds to cereblon to degrade transcription factors IKZF1/IKZF3. Thalidomide has been reintroduced for use in multiple myeloma, erythema nodosum leprosum, some skin disorders, HIV-associated wasting syndrome, among others[6][7][9]. This specific five-drug combination does not correspond to any known approved or branded product but may be considered or studied experimentally for severe cachexia/wasting syndromes due to its components' synergistic effects on appetite stimulation, inflammation modulation, hormone replacement/antagonism, immune modulation, or cancer therapy.
