L-carnitine + metformin is a combination of two small molecule agents, each with distinct but potentially complementary mechanisms. L-carnitine (also known as levocarnitine) is a naturally occurring compound that facilitates the transport of long-chain fatty acids into mitochondria for beta-oxidation and energy production. It plays an important role in lipid metabolism and cellular energy homeostasis[5][3]. Metformin is a biguanide class oral antihyperglycemic agent widely used to treat type 2 diabetes; it decreases hepatic glucose production, reduces intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake[4][6]. The combination has been studied for synergistic effects on metabolic disorders such as obesity and polycystic ovary syndrome (PCOS), where it enhances brown adipose tissue activity, promotes white adipose tissue browning via the Nrf2/heme oxygenase-1 pathway, reduces inflammation, improves insulin resistance, and positively affects reproductive outcomes in clomiphene-resistant obese PCOS women[1][2].
