Overview

This is a combination of two small molecule drugs, l-deprenyl (also known as selegiline) and physostigmine. L-deprenyl is a selective monoamine oxidase B (MAO-B) inhibitor primarily used in the treatment of Parkinson's disease and has been studied for cognitive effects in Alzheimer's disease. Physostigmine is a reversible cholinesterase inhibitor that increases acetylcholine levels at cholinergic synapses and is used to treat glaucoma and anticholinergic toxicity. The combination has been investigated for potential additive or synergistic effects on cognition in Alzheimer's disease, but clinical studies have not demonstrated significant improvement over placebo when used together[1][2]. Both agents act via distinct mechanisms—l-deprenyl by inhibiting dopamine breakdown through MAO-B inhibition, and physostigmine by preventing acetylcholine degradation.
