**L-DOS47 + pemetrexed + carboplatin** is an experimental combination regimen for cancer therapy, specifically studied in patients with advanced or metastatic nonsquamous non-small cell lung cancer (NSCLC). L-DOS47 is an immunoconjugate consisting of a camelid single-domain antibody targeting carcinoembryonic antigen-related cell adhesion molecule 6 (CEACAM6), conjugated to urease. It acts by localizing urease to tumor sites, where it converts urea into ammonia and carbon dioxide, raising the pH of the acidic tumor microenvironment and causing cytotoxicity. Pemetrexed is an antifolate antineoplastic agent that inhibits key enzymes in folate metabolism, thus hindering DNA and RNA synthesis in rapidly dividing cells. Carboplatin is a platinum-based chemotherapeutic that induces DNA crosslinking and apoptosis. The combination has shown additive cytotoxicity in vitro and demonstrated safety and clinical benefit in early phase trials for NSCLC.
