L-PRF + metronidazole is a combination therapy consisting of leukocyte platelet-rich fibrin (L-PRF), an autologous biomaterial derived from centrifuged blood that contains platelets and leukocytes, and the antibiotic metronidazole. L-PRF acts as a scaffold for tissue regeneration and has inherent antimicrobial, anti-inflammatory, and wound-healing properties. When loaded or combined with metronidazole—a nitroimidazole-class antibiotic effective against anaerobic bacteria—the resulting matrix serves as a local drug delivery system for targeted antimicrobial action. This combination is primarily investigated as an adjunctive treatment in periodontal therapy to enhance clinical outcomes by reducing pathogenic bacterial load (notably Porphyromonas gingivalis) and promoting periodontal healing[1][3]. The mechanism involves both the regenerative effects of growth factors released from L-PRF and the bactericidal activity of locally delivered metronidazole.
