Overview

Lactated Ringer's + Plasma-Lyte refers to the combined or sequential use of two balanced crystalloid intravenous fluids, each designed to closely mimic the electrolyte composition of human plasma. Both are used for fluid and electrolyte replenishment in clinical settings such as surgery, trauma, dehydration, and critical illness. **Lactated Ringer’s** is an isotonic solution containing sodium (130 mEq/L), chloride (109 mEq/L), potassium (4 mEq/L), calcium (2.7–3 mEq/L), and lactate (28 mEq/L) as a buffer[2][5][6]. The lactate is metabolized in the liver to bicarbonate, helping correct metabolic acidosis but can transiently increase serum lactate levels[3]. Its osmolarity is about 273–272 mOsmol/L with a pH around 6.5–6.7[2][5][6]. **Plasma-Lyte A** is another isotonic balanced crystalloid containing sodium (140 mEq/L), chloride (98–103 mEq/L), potassium (5 mEq/L), magnesium (3 mEq/L), acetate (27 mEq/L) and gluconate (23 mEq/L)[4][5][1]. Acetate and gluconate serve as buffers that are rapidly metabolized to bicarbonate without increasing serum lactate levels[1][4]. Its osmolarity is about 294 mOsmol/L with a pH of 7.4[4][5]. Both solutions are preferred over normal saline for large-volume resuscitation due to their lower risk of causing hyperchloremic metabolic acidosis; however, they differ in buffering agents—lactated Ringer’s uses lactate while Plasma-Lyte uses acetate/gluconate—and in their calcium/magnesium content[1][3][7].
