Lactated Ringer's solution (LR) is a sterile, isotonic, physiologically balanced crystalloid intravenous fluid used primarily for fluid resuscitation, volume substitution, and electrolyte replenishment. It is composed of sodium chloride, sodium lactate, potassium chloride, and calcium chloride in water. Unlike normal saline, lactated Ringer's solution contains sodium lactate, which is metabolized by the liver into bicarbonate, acting as a buffering agent to mitigate metabolic acidosis. In the context of acute pancreatitis (AP), lactated Ringer's solution has been investigated in multiple clinical trials (such as the WATERFALL and WATERLAND trials) led by investigators like Enrique de Madaria. Evidence suggests that LR may exert anti-inflammatory effects in AP compared to normal saline, potentially reducing systemic inflammatory response syndrome (SIRS), C-reactive protein (CRP) levels, and overall disease severity. The solution is manufactured as a generic product by multiple pharmaceutical companies, including Baxter, B. Braun, and Fresenius Kabi.
