Lactic acid + erythromycin is a topical antimicrobial combination developed by Beijing Yanhua Hospital for the management of wounds sustained during potential rabies exposure. Erythromycin, a macrolide antibiotic, exerts its effect by binding to the 50S subunit of bacterial ribosomes, thereby inhibiting protein synthesis and preventing secondary bacterial infections common in animal bite wounds. Lactic acid is utilized for its antiseptic properties and its ability to maintain an acidic environment, which is detrimental to many pathogens. This formulation is primarily used for thorough wound irrigation as part of a comprehensive post-exposure prophylaxis (PEP) regimen to reduce viral and bacterial load at the site of injury.
