Overview

**Lactobacillus plantarum strains 299 and 299v** is an orally administered live probiotic preparation evaluated by the Children's Oncology Group as an investigational microbiome therapy for prevention of gastrointestinal acute graft-versus-host disease after alternative-donor allogeneic hematopoietic cell transplantation in children and young adults. The preparation contains two genetically similar strains, *Lactobacillus plantarum* 299v (DSM 9843) and *Lactobacillus plantarum* 299 (DSM 6595), now classified as *Lactiplantibacillus plantarum*. It is intended to support intestinal mucosal integrity and beneficial microbiome composition, potentially reducing gastrointestinal inflammation and susceptibility to acute graft-versus-host disease. In the randomized Phase 3 ACCL1633 trial, dosing began with conditioning and continued through day 56 after transplantation; the product was well tolerated without reported *Lactobacillus* bacteremia but did not significantly prevent gastrointestinal acute graft-versus-host disease versus placebo.
