BQ-L14 is a 14-day bismuth-based quadruple therapy regimen utilized for the first-line eradication of Helicobacter pylori infection. The regimen consists of the proton pump inhibitor (PPI) lansoprazole (30 mg twice daily), the mucosal protective and antimicrobial agent bismuth tripotassium dicitrate (300 mg three times daily), and two antibiotics: tetracycline (500 mg three times daily) and metronidazole (500 mg three times daily). This combination is particularly effective in clinical settings characterized by high clarithromycin resistance. Lansoprazole suppresses gastric acid secretion by inhibiting the H+/K+-ATPase pump, thereby creating an optimal pH environment for antibiotic activity. Bismuth tripotassium dicitrate provides direct bactericidal effects against H. pylori and offers cytoprotective properties to the gastric mucosa. Tetracycline and metronidazole exert broad-spectrum antibacterial activity by inhibiting bacterial protein synthesis and disrupting DNA, respectively.
