lansoprazole + metronidazole + clarithromycin

Lansoprazole + metronidazole + clarithromycin is a triple-therapy combination of small molecule drugs used to eradicate *Helicobacter pylori* infection and treat peptic ulcer disease. Lansoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by irreversibly blocking the H+/K+ ATPase enzyme (the 'proton pump') in gastric parietal cells, thereby creating a less acidic environment that enhances the efficacy of the antibiotics. Clarithromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, while metronidazole is a nitroimidazole antibiotic that disrupts bacterial DNA synthesis. Together, the three agents act synergistically to eradicate *H. pylori*. This combination is being studied in a Phase 4 randomized trial (the HEAT study) sponsored by the University of Nottingham to assess whether eradication therapy reduces hospitalizations for peptic ulcer bleeding in older aspirin users compared with no eradication treatment.
