Lapaquistat acetate + atorvastatin is a combination therapy investigated for the treatment of hypercholesterolemia. Lapaquistat acetate is a squalene synthase inhibitor that blocks cholesterol biosynthesis at a step distal to HMG-CoA reductase, while atorvastatin is an HMG-CoA reductase inhibitor (statin) that reduces cholesterol synthesis earlier in the pathway. The combination was studied to assess whether dual inhibition would provide additive or synergistic lipid-lowering effects. Clinical trials demonstrated that co-administration significantly reduced low-density lipoprotein cholesterol (LDL-C), total cholesterol, non-HDL-C, apolipoprotein B, triglycerides, and high-sensitivity C-reactive protein compared to monotherapy. However, development of lapaquistat acetate was terminated due to hepatic safety concerns observed in late-stage clinical trials[1][2][3][6].
