Overview

This is a combination therapy involving the use of a pulsed dye laser (PDL) and topical rapamycin (also known as sirolimus), primarily investigated for the treatment of capillary vascular malformations such as port-wine stains (PWS), including in patients with Sturge-Weber syndrome. The rationale is that while PDL is the gold standard for treating these vascular lesions, recurrence due to revascularization is common. Rapamycin acts as an mTOR inhibitor with antiangiogenic properties, reducing vascular endothelial growth factor (VEGF) and other proangiogenic signals to inhibit blood vessel regrowth after laser-induced injury. Clinical studies have shown that combining topical rapamycin with PDL can improve outcomes compared to either treatment alone by minimizing post-laser revascularization and enhancing lesion blanching. Some studies also explore enhanced drug delivery systems like Tixel to further increase skin permeability and efficacy of topical rapamycin application[1][2][3][4][5][6].
