LAVA-1207 + pembrolizumab is an investigational combination therapy for metastatic castration-resistant prostate cancer (mCRPC). LAVA-1207 is a humanized bispecific antibody that binds with high affinity to the Vδ2 chain of Vγ9Vδ2-T cells and to prostate-specific membrane antigen (PSMA) on tumor cells. This dual binding leads to potent activation and cytolytic activity of Vγ9Vδ2-T cells against PSMA-expressing prostate cancer cells. Pembrolizumab, marketed as Keytruda, is a monoclonal antibody targeting the PD-1 immune checkpoint receptor, thereby enhancing T-cell mediated anti-tumor responses. The rationale for combining these agents is that pembrolizumab may potentiate the effect of LAVA-1207 by inhibiting PD-1-mediated suppression of T cell activity in the tumor microenvironment[1][2][3][8]. The combination has been evaluated in phase 1/2a clinical trials for safety, pharmacokinetics, pharmacodynamics, immunogenicity, and preliminary antitumor activity in patients with refractory mCRPC[3][5].
