LBL-024 + gemcitabine + cisplatin is an investigational combination regimen for cancer therapy, particularly in advanced extrapulmonary neuroendocrine carcinoma (EP-NEC). LBL-024 is a bispecific antibody that simultaneously targets PD-L1 (programmed death ligand 1), blocking this immunosuppressive pathway, and provides 4-1BB (CD137) costimulation to enhance antitumor T cell activity within the tumor microenvironment. It is studied in combination with gemcitabine (a nucleoside analog antimetabolite that inhibits DNA synthesis) and cisplatin (a platinum-based DNA cross-linking agent), both chemotherapeutic agents widely used and synergistic in multiple solid tumors. The first-in-class combination aims to synergize direct cytotoxic effects from chemotherapy and checkpoint/costimulatory immunomodulation from LBL-024, resulting in robust immune-mediated and cytotoxic antitumor activity. Clinical trials have demonstrated high response rates and disease control in EP-NEC, supported by breakthrough and orphan designations for LBL-024 in neuroendocrine cancers.
