**LBL-024 + pemetrexed + carboplatin** is a combination therapy being investigated for the treatment of advanced extrapulmonary neuroendocrine carcinoma (EP-NEC) and potentially other cancers. LBL-024 is a novel bispecific antibody that simultaneously targets programmed death-ligand 1 (PD-L1) and 4-1BB, providing both checkpoint inhibition and conditional co-stimulation to enhance anti-tumor immunity. Pemetrexed is an antimetabolite chemotherapy agent that interferes with folate-dependent metabolic processes essential for cell replication, while carboplatin is a platinum-based chemotherapy that induces DNA crosslinking and apoptosis in rapidly dividing cells. The combination leverages the immune-activating effects of LBL-024 together with the cytotoxic effects of pemetrexed and carboplatin for synergistic anti-cancer activity. Early-phase clinical trial results in first-line, treatment-naive advanced EP-NEC patients showed high response rates and manageable safety, notably outperforming historical outcomes with chemotherapy alone.
