Lenacapavir in combination with an optimized background regimen (OBR) is a therapeutic approach developed by Gilead Sciences for the treatment of multi-drug resistant HIV-1 infection. Lenacapavir is a first-in-class, long-acting HIV-1 capsid inhibitor that disrupts the viral lifecycle at multiple stages, including capsid-mediated nuclear uptake of viral DNA, virus assembly and release, and capsid core formation. The optimized background regimen (OBR) consists of a personalized combination of antiretroviral agents (such as NRTIs, NNRTIs, PIs, or INSTIs) selected by a clinician based on the patient's specific viral resistance profile and treatment history. This combination is particularly indicated for heavily treatment-experienced (HTE) patients, including adults, adolescents, and children, who have limited treatment options due to extensive drug resistance.
