Drug intelligence / Profile preview
Lenalidomide + cyclophosphamide + dexamethasone is an all‑oral three‑drug chemotherapy/immunomodulatory regimen used primarily for the treatment of multiple myeloma, including newly diagnosed, relapsed/refractory, and transplant‑ineligible patients, and has also been studied in systemic AL amyloidosis.[1][3][7][9] Lenalidomide is an immunomodulatory small molecule structurally related to thalidomide that enhances T‑cell and NK‑cell activity, inhibits pro‑myeloma cytokines, and exerts direct antiproliferative and pro‑apoptotic effects on myeloma cells.[1][7] Cyclophosphamide is an alkylating prodrug that forms DNA crosslinks, leading to inhibition of DNA replication and apoptosis of rapidly dividing plasma cells.[1][3] Dexamethasone is a synthetic glucocorticoid that induces apoptosis of myeloma cells and synergizes with lenalidomide and cyclophosphamide to deepen responses.[1][3][7] Clinical studies have shown this combination to be highly active with high overall response rates and manageable toxicity, though with increased hematologic toxicity (notably neutropenia and thrombocytopenia) compared with lenalidomide–dexamethasone alone.[1][3][6][7]
